(CLA) Tutor Program Agreement

As a parent, I understand that my child is responsible for the progress he/she makes, while in the program. This will be dependent on how hard my child works and by the attitude displayed. I understand that my tutor will lend assistance, but will not do the work for my child.

I Agree:
  • that my child should attend all scheduled tutor sessions.
  • to services lasting for an entire school year.
  • to special holidays being recognized and honored.
  • to signing my child in and out at every face-to-face session.
PLEASE NOTE: If you have to cancel, we request that you do so at least 24 hours, before a scheduled class. However, if you do not cancel prior to the 24 hours, you will lose the full payment for the class. We do not offer refunds.

No Liability Insurance and Acknowledgments

My signature confirms that I understand that I assume ALL RISKS when my child receives tutoring. This includes healthcare fees, attorney fees, etc. During virtual sessions, Carthagena Learning Academy is not liable for any inappropriate content that your child may log into; therefore, please monitor your child's device to ensure parental guidance is turned on.

Photography Authorization

Carthagena Learning Academy will occasionally take photographs and/or videos throughout our facility during special events, engaging activities, and for general classroom use. These photographs may be displayed around the center, in the classroom, in newsletters, or on our website.

Pricing

Registration Fee

There is a mandatory non-refundable registration fee of $50 per child. This is due once every school year and it must be paid in full upon enrollment or re-enrollment. Registration Fees will NOT be used to purchase books or manuals.

Withdrawal/Termination

A two weeks notice of intent to withdraw is requested. No refunds will be issued.

Methods of payment: Cash/Debit/Credit/Money Order (There will be a $35 fee for any returned checks)

Important: You must complete all required agreements, including checking all agreement boxes, selecting photography permission, and providing your name and date before you can access the Student Information section.
